Granville Academy Northern Virginia Media Release Form, in PDF format, gives permission to the Granville Academy Northern Virginia to release your name to representatives of the media, to include, radio, newspapers, and television, for the purposes of being interviewed regarding the Granville Academy program.
Medical Information & Permission for Emergency Care Form, gives the Granville Academy Northern Virginia your permission, in an emergency when you or your physician cannot be contacted, to take your child to the emergency room of the nearest hospital, and the hospital and its medical staff have your authorization to provide treatment which a physician deems necessary for the well-being of your child.
Further, it gives Granville Academy Northern Virginia your permission to dispense your child’s medication and other medication (aspirin, Tylenol, Advil) deemed necessary for common ailments like headaches & stomach aches.
Granville Academy Northern Virginia Affiliate Travel Authorization and Expense Voucher, is used to track related expenses when attending Academy sponsored events.
